Please note: All forms below are in PDF format and require Adobe Acrobat Reader installed on your computer. You can download Acrobat Reader for free at Adobe's Web Site.

  1. Complete the Application for Enrollment form. Please mark the appropriate program.
    • Download IEP Application for Enrollment (PDF)
  2. Complete the Application for I-20 form (all F-1 students).
    • Download I-20 Student Visa Application Form (PDF)
  3. Complete the Affidavit of Support (all F-1 student)
  4. Attach official TOEFL score results (Graduate Preparation-Level Six and University Study program applicants).
  5. Enclose a check or international bank draft in the amount of US $100 (non-refundable application fee).
  6. Enclose a check or international bank draft for housing placement fees, if applicable.
  7. Mail completed application materials along with the application fee to: International Extension Programs; College of Extended Learning; California State University, San Bernardino; 5500 University Parkway, Room SH-134; San Bernardino, CA 92407-2393 U.S.A.
